I would imagine it would have been hard to get her to cooperate, lie still, etc during such a horrible experience as an abortion. No doubt that would not deter someone like a diehard abortionist from going ahead anyway. She probably tried to get away from them and was injured in the process. They probably didn't want any of their patients to get away from them, so they just went ahead even though she didn't lie still.
